DISCLOSURE & RELEASE FORM

 I_____________________________________ am in full understanding that BO Allen has attempted to bite/nip on more than one occasion.  While he has not made contact or broken skin we feel it would be best that he go to a home with adults. We feel a quieter home without as much activity will be best, as a home with children and lots of people entering and exiting the home would likely trigger this behavior. After observing him in foster care we feel some of the behavior is breed based, he is a dog who is intelligent and requires both mental and physical stimulation, likely would excel at adgility. 

Since moving him to a foster home that meets this description we have not seen any of the behavior previously observed. Bo is a loving and loyal pet who will make a great companion, and will thrive in the right home environment.

We recommend feeding animals in their crates and keeping food, toys and treats out of play if there are other dogs or children in the home. Bo Allen would benefit from a structured environment that employs the recommendations provided in our transition tips in helping him acclimate to his new home.

LCAR has fully disclosed the information known about Bo Allen.  In consideration for being permitted to foster or adopt Bo I assume full responsibility for the risk of bodily injury or property damage caused by this dog.  

By signing this form, I have relinquished any liability of LCAR and fully indemnify its officers, members and volunteers from any loss, liability, damage or cost that may be incurred on account of this dog.
